Advance Directives
When you are admitted to Glendale Memorial Hospital, you will receive information about Advance Healthcare Directives. These Healthcare Directives (Power of Attorney For Health Care Decisions) allow you to make your wishes known regarding healthcare decisions and/or appointing another person who will make decisions for you if you are not able.

Physicians and/or hospital staff may be unable to honor your Advance Directive based on conscience and/or conflict with accepted healthcare standards. In these cases, you will be promptly notified so that options for care and treatment can be addressed.

Bioethics Committee
Glendale Memorial Hospital and Health Center strives to maintain exceptional levels of ethical conduct and behavior. We conduct our services with integrity and stand by our mission to "improve the health and quality of life of the people we serve."

We recognize that advances in medical science and technology have contributed to increasingly difficult and complex bioethical, legal and social questions concerning health care and that medical staff, clinical staff, families and the community need resources to assist in addressing these ethical issues.

Glendale Memorial Hospital's Bioethics Committee is available to all patients and their families. The committee consists of healthcare professionals, administration, spiritual care advisors and community representatives to assist patients and their family members with resources for resolving these decisions. If you have questions please contact your unit social worker or nurse manager.
